With each shipment you will receive a postage-paid mail order form and confirmation of your current order. Simply complete the information, sign and date the form, and mail it back.
Phone Message Order
Call 1 800 343 4944 and follow the prompts. When you hear the tone, leave the following information:
*The patient's first and last name
*The patient's reorder number
*The patient's area code and phone number
*Any changes to the order, product or quantity.
Phone Order with a Representative
Call 1 800 343 4944 and press "1" for the supply department. Wait on the line for the next available representative to take your order.
Once you place your order, you should receive your shipment within 5 business days.
